How do you covert from PDF's to epub/mobi without any formatting issues

Hi all,
I use calibre to convert PDF's to epub/mobi but the result is not perfect i see a lot of random spaces and formatting is totally off when i use it in my kindle 4. Is there any setting that i should be aware of or do you guys have any other alternative to calibre ?

I typically use online-ebook-convert but there will always be formatting issues

Migtht sound lame, but maybe M$ Word could help you here. Convert to a doc or html first.

If your not worried about searching text, maybe convert the pdf to tiff images and then convert to whatever format you want.
